The Diaz Cartel is run by the Diaz Brothers and control Little Havana at the start of the game. They rise to power after the mansion shootout, similar to other gangs. They tend to avoid open conflict unless forced to, being the only gang with such a reputation. However, they are twice as sadistic to those who cross them than other gangs, as evidenced from them killing Georgina Montana, just to spite Tony.

When Tony returns, he sets his eyes on Little Havana and constantly attacks Diaz gang nests, forcing the remaining members to flee to the storehouse and to Diaz Motors (assuming all of the nests have already been eliminated). The Diaz Brothers then order a hit on Tony and his mother as revenge. Tony is attacked at the Babylon Club, but survives. He interrogates the leading assassin, Mr. Smith and learns of his mother's death. In a rage, Tony kills Smith and then decides to butcher the Diaz brothers at Diaz Motors with a chainsaw. After slaughtering the members guarding the Motors and Edgar, Tony chases in a car after Alfonso Diaz and kills him as well, either with a gun or chainsaw. Sometime later, Montana, Tino and their crew take over the Havana storehouse, possibly wiping out the cartel for good. However, it can be assumed that some of the members survived and moved to Downtown, as they can be seen as enemies in Felix's leads, especially in missions concerning the Blood Brothers, whose base is in Downtown.

- Diaz Cartel is one of the more diversed cartels in the game, as its leaders are American-Colombian, while their gangsters appear to be of African-American, Hispanic and Jamaican descent.
- They are the only gang in the game that are not fought in any of the front missions in their respective territory, minus the Havana Storehouse.
