Tom Gaffney was the protoge of O'Hara, and his right hand man in The North Side gang. After O'Hara was killed under Tony Camonte's orders during his crusade of taking over the north side, Gaffney assumed control of the gang and went to war with the Southsiders. However, Tony and his men fought back hard, and Gaffney was forced to go into hiding with his gang. Eventually, Tony and his men tracked down gaffney to a bowling alley, and killed him.

- Gaffney is based off both Hymie Weiss and Bugs Moran, Irish gangsters who had a bloody rivalry with Al Capone, whom Tony Camonte is based off of.
- He is portrayed by Boris Karloff, who is most well known for playing the monster in the 1931 film Frankenstein.
